We have the flu and Covid (pfizer/ biontech) vaccine!  We are OUT OF NETWORK with all insurances. The rates are listed below. Telehealth visits, while not ideal, are available upon request for established patients. 

BLOOD WORK:  We draw in the office and send to BIOREFERENCE. Or, we can give you a slip to take to Bioreference or LABCORP.

• Well visit blood work is typically covered once per year and often includes: lipid panel, blood count (anemia) thyroid screen (TSH), electrolyte/ liver/kidney panel (CMP), and STD tests. Vitamin D and B12, and a diabetes screen (a1c) are often NOT covered, and may fall under your lab deductible/ coinsurance policies. You are responsible for knowing what is covered and what is not.  
• If you do not update your insurance with us, then insurance will deny blood work, and the lab will bill you. Insurance numbers often change, even if it is the same insurance company, especially in a new calendar year, so let us know.
• Fasting is needed primarily for an accurate cholesterol panel– PLEASE drink water and hydrate. If you get light headed with blood work, we encourage you to hydrate before and eat something light, like a piece of fruit, which will not affect it. Hold vitamins on the day of the blood work as well. 
• If you do not have insurance, we will arrange for insurance based (very reduced) prices for you, estimate of the self pay lab prices. 

If You Have Out of Network Benefits with Aetna, Cigna, United/ Oxford, we will submit your claims, which run up your deductible, or might even pay for part or all of the visit. If they reimburse us, we CREDIT your visit so that the next visit, we will not bill you. We do NOT provide insurance with records to prove our services. We DO NOT submit claims for HMOs, BCBS, Medicare (we have opted out) or smaller insurances.  FSA and HSA cards are accepted.  

RATES:

• Established Patient Visit:  18-39 yrs old $200. 40-64 yrs old $250. 65+ $350. (If not seen in over 3 years, then will charge New Patient Visit Rates).
• New Patient Visit:  18-39 yrs old $400. 40-64 yrs old $600. 65+ $800.
• Phone or Email Evaluations that Lead to a Prescription or Diagnostic Test:  $150
• Close follow ups for Established Problems:  $150
• Blood Draw:  $25. Blood Draw for Outside Providers, Outside Tests $150. 
• Urine testing $25 
• Rapid Flu, or Strep, or Covid $50
• Vaccine administration $40
• Vaccines:  Typhoid $220. HPV $450. Pneumonia/shingles $300. Tdap $75. Hep A/B, $120. Flu $40. Meningococcal $210. Japanese encephalitis $500. MMR $200. Pfizer Covid $175.
• EKG $150
• Spirometry $150
• Impacted Ear Wax Removal $125 per each side.
