We are OUT OF NETWORK with all insurances. The rates are listed below. Telehealth visits, while not ideal, are available upon request for established patients.

Blood Work is done in the office and sent to Quest. Our network status is independent of your lab work, radiology, and medication coverage. You are responsible for knowing your coverage. Well visit blood work is typically covered once per year and includes:  lipid panel, thyroid screen (TSH), electrolyte/ liver/kidney panel (CMP), STD tests, and a diabetes screen (a1c). Vitamin D and B12 are covered about half the time, and may fall under your lab deductible/ coinsurance policies. We provide your insurance information to Quest. If you do not update your insurance with us, then you will get a large bill from Quest as your insurance will deny it. Insurance numbers often change, even if it is the same insurance company, especially in a new calendar year, so let us know. 

Fasting is only needed for an accurate cholesterol panel– PLEASE drink water and hydrate. If you get light headed with blood work, we encourage you to hydrate before and eat something light, like a piece of fruit, which will not affect it. Hold vitamins on the day of the blood work as well. You can take meds with water. 

RATES:  If you have out of network benefits with Aetna, Cigna, United/ Oxford, we submit your claims, which runs up your deductible. If they reimburse us, we will credit your visit so that the next visit, we will not bill you. We do not submit claims for HMOs, BCBS, Medicare (we have opted out) or smaller insurances– you can request a bill that you can submit to your insurance directly. FSA and HSA cards are accepted.  

• New Patient Visit:  18-39 yrs old $350. 40-64 yrs old $500. 65+ $750.
• Established Visit:   18-39 yrs old $200. 40-64 yrs old $250. 65+ $350.
• Phone/ Email evaluations that lead to a prescription or diagnostic test. Close follow ups $150-300 (for established patients only)
• Urine testing $25

• Vaccines: Typhoid $150. HPV $350. Prevnar/shingles $250. Tdap $75. Hep A/B, $100. Flu $40
